For patients at our Oregon clinics, call or text us at 541-386-6380 for a MyChart activation code. For patients at our Washington clinics (formerly NorthShore Medical Group), call us at 509-493-2133 for White Salmon and 509-427-4212 for Stevenson.
Parents or guardians can see their child’s health record in MyChart from birth until the child turns 12.
- When the child turns 12, full access changes to limited access.
- Access ends when the child turns 18. At that time, the patient can add another adult proxy if needed.
