Youth Proxy

A parent/guardian of a child between ages 0 up to their 12th birthday has full access to the child’s health record via MyChart. Full proxy access automatically converts to limited access when a child turns 12. Proxy access ends on the patient’s 18th birthday, at which time they may add another adult proxy if needed.

Under Oregon law, minors have a right to consent to their own age-appropriate reproductive healthcare at any age, and can consent to access mental healthcare at age 14 and general medical care at age 15. While confidentiality is a legal right, youth providers know the value of parent/guardian involvement in the health care of their children. Clinical staff talk regularly with youth about how to involve their support systems in medical decision-making, and how to increase parent/child communication. Through conversations with providers and family members, youth are empowered to understand how healthcare systems work and how to access them when they need mental or medical healthcare, which helps build important skills that will support them as they become independent adults. School-based health service providers work hard to center the needs of youth and their loved ones while consistently upholding best practices under Oregon law.

Understanding MyChart Proxy for Youth

Depending on the child’s age, parents and guardians have different levels of access to their child’s MyChart information:

Parents and guardians have full access to all MyChart features through their own MyChart log in. A child age 11-years or younger cannot have a personal MyChart account.

A parent/guardian of a child between ages 0 up to their 12th birthday has full access to their child’s health record via MyChart.

This includes the ability to:

  • Manage appointments
  • Get test results
  • Request or renew prescriptions
  • View after-visit summaries
  • Send secure messages to child’s provider
  • Pay bills online (if also the guarantor)

Full proxy access automatically converts to limited access when a child turns 12. This is due to state laws that protect a teen’s health information privacy and their right to seek certain confidential health care services without a parent/guardian’s consent.

Proxy access ends on the patient’s 18th birthday, at which time they may add another adult proxy if needed. (See below for more information)

Limited access of health record includes the ability of a parent/guardian to:

  • View Patient’s Allergy Information

  • View Patient’s Inbox Messaging and Provider Messaging

Youth can sign up for their own MyChart account during their clinic visit.

A parent or guardian may be granted full access to a teen patient’s account under certain circumstances, such as when a patient has severe cognitive disabilities or is undergoing treatment for a serious condition like cancer.

Ask your teen’s provider for details.

New to MyChart? If you are requesting proxy access another person’s MyChart account, you will need to create your own MyChart account at One Community Health.

It is easy to create a One Community Health patient record for yourself.

Adults (age 18 and older) can authorize another person to access their MyChart information to help with their health care needs. Proxy access can be stopped at any time by the proxy or the patient.

Levels of Adult Proxy Access:

  • Full Access: Access to all MyChart features.
  • Incapacitated Adult: For adults who are not able to make their own medical decisions, proxy access can be provided to the legal representative.

For an adult or child who is not able to make his or her own medical decisions, full access can be provided to the legal representative.

This form is required for:

Adult Accessing Adult: Patients 18 and older may assign proxy access to another adult. Examples of Adult Accessing Adult proxies are spouses, family member/adult children of elderly parents, or caregiver access.

Diminished Capacity : A Diminished Capacity proxy may be assigned to all patients as needed and appropriate.

Children 11 years and younger cannot have a personal MyChart account. Their medical information will be linked to the proxy’s account.

If you have access to your family members’ medical records, you can view most of the information in their records in the same way that you view your own.

After you’ve received permission, view other information in your family member’s record by switching to their chart.

From the Switch menu, select the appropriate family member’s name

If you’re a parent/guardian, you can have access to the child’s record through age 11 (until the 12th birthday) and limited access through age 17 but must be added by the child.

For those between the age of 12 and 17, you can give access to a parent or guardian by following the below steps.

1. You can grant a friend or family member to your MyChart account from Menu>Sharing> Sharing Hub activity.

2. Select the resource you want to share your chart with.

3. Decide how you want the information to be shared.
