School Sports Physicals

Before your child can play a sport, the state requires a sports physical to be completed by a medical provider. Physicals ensure a child’s safety and ability to participate in sports. During these visits you can also discuss physical and mental health, check their immunization records, and ask any health-related questions. We offer sports physical appointments year-round at all of our clinics, including our school-based health centers.

This year One Community Health is offering several sports physical days in collaboration with Gorge middle and high schools.

All students planning to participate in school sports for the 2025-2026 school year need a sports physical prior to joining the team.

Come in to cross sports physical exam off your summer to do list.

Don’t forget to bring a Sports Physical form signed by a parent/guardian.

For Wasco county, call/text 541.386.6380 to schedule at our clinic in The Dalles or 541.386.2304 for students at Dufur School.

On the day of the appointment, don’t forget to bring a Sports Physical form signed by a parent/guardian.
